Job Description & RequirementsRN Case ManagerStartDate: 2/3/2025 Pay Rate: $2000.00 - $2700.00POSITION SUMMARYRN Case ManagerPOSITION DUTIESThe Registered Nurse Non-Certified Case Manager works with physicians and multidisciplinary team members to develop a plan of care for assigned patients. Ensures patient is progressing towards desired outcomes by monitoring care through assessments and/or patient records. Identifies and resolves barriers that hinder effective patient care. Actively involved in discharge planning process.MINIMUM REQUIRED QUALIFICATIONSMontana Licensure (Registered Nurse) or Compact (Multistate)American Heart Association BLSMinimum of 2 years of nursing experiencePREFERRED QUALIFICATIONSBachelor's Degree in Nursing2 years of acute care hospital experience as a case managerLENGTH OF ASSIGNMENT13 WeeksSHIFT / HOURS PER WEEKDay Shift. 730am -430pm, Weekend Rotations The position may necessitate working beyond a standard 40-hour workweek, including weekends and after-hours shifts. On-call work may be required to respond promptly to organizational, patient, or employee needs.
