Job Title : Case Manager Registered Nurse (RN)**\n Job Summary :\n
A Case Manager Registered Nurse (RN) coordinates and manages the
care of patients, ensuring they receive appropriate,
cost-effective, and timely healthcare services. Case managers work
across various healthcare settings, including hospitals, outpatient
facilities, and home care, to facilitate patient care transitions,
advocate for patients, and collaborate with multidisciplinary teams
to promote optimal outcomes.
\n\n Key Responsibilities :\n \n
Conduct comprehensive assessments of patients' medical,
psychosocial, and financial needs to develop individualized care
plans. \n
Coordinate and facilitate care across multiple healthcare settings,
ensuring continuity of care, appropriate utilization of resources,
and adherence to treatment plans. \n
Collaborate with physicians, healthcare providers, and insurance
companies to authorize treatments, secure necessary services, and
ensure compliance with clinical guidelines. \n
Evaluate patient progress, monitor outcomes, and adjust care plans
based on patients' evolving needs and goals. \n
Educate patients, families, and caregivers on disease management,
treatment options, and available resources to support recovery and
self-care. \n
Advocate for patients to ensure they receive appropriate services
and that care is delivered efficiently and effectively. \n
Facilitate communication among healthcare providers, patients, and
insurance companies to resolve barriers to care. \n
Assist patients with obtaining necessary medical equipment,
medications, and follow-up care to ensure successful transitions
post-discharge. \n
Track healthcare costs, utilization of services, and outcomes,
contributing to cost containment strategies and improved care
delivery. \n
Monitor and ensure compliance with healthcare regulations,
insurance policies, and external mandates related to case
management. \n
Participate in quality improvement initiatives, utilizing
evidence-based practices to enhance patient care. \n
Provide guidance and support to patients and families navigating
complex healthcare systems. \n
\n\n Qualifications :\n \n
Education : Bachelor of Science in Nursing (BSN) preferred;
Associate Degree in Nursing (ADN) accepted. \n
Licensure :\n \n
Valid Registered Nurse (RN) license in the state of practice.

Certification :\n \n
Case Management certification (CCM) preferred. \n
Basic Life Support (BLS) certification required. \n
\n \n
- Experience : Previous experience in case management, care
coordination, or clinical nursing, particularly in
medical-surgical, home health, or specialized units \n
