Job Title: Travel RN - Case Manager Job Type: Contract /
Temporary (Travel Assignment) Location: Various locations across
the country (travel required) Job Summary: We are seeking a skilled
and compassionate Registered Nurse (RN) with experience in Case
Management to join our team on a travel nursing basis. The ideal
candidate will be responsible for coordinating patient care across
multiple settings, ensuring that patients receive appropriate and
timely services, and helping them navigate the healthcare system.
This role involves working closely with interdisciplinary teams to
develop, implement, and monitor care plans while promoting patient
health and advocating for their needs. Key Responsibilities:
Develop and manage comprehensive care plans for patients, ensuring
continuity of care across various settings (hospital, home health,
outpatient, etc.). Conduct patient assessments to evaluate needs,
identify potential issues, and develop individualized care plans.
Coordinate care with physicians, specialists, social workers, and
other healthcare professionals to ensure a holistic approach to
patient care. Monitor and track patient progress, adjusting care
plans as necessary to optimize outcomes. Advocate for patients by
ensuring they have access to appropriate medical resources,
services, and support. Educate patients and families on treatment
options, healthcare services, and resources available to them.
Facilitate communication between healthcare providers, patients,
and family members to ensure the care plan is understood and
followed. Ensure adherence to healthcare regulations, policies, and
standards, including those related to HIPAA, patient privacy, and
safety. Provide emotional support to patients and families, helping
them cope with complex health issues. Travel to various healthcare
facilities to manage cases and provide support in different
clinical settings. Qualifications: Required: Active RN license in
good standing (State-specific) and ability to obtain licensure in
other states as required. Minimum of 3-5 years of clinical nursing
experience, with at least 2 years in case management or a related
field. Strong knowledge of healthcare systems, insurance processes,
and case management practices. Ability to assess patient needs,
develop care plans, and collaborate with multidisciplinary teams.
Strong organizational and time-management skills to handle multiple
cases and competing priorities. Excellent communication skills,
both written and verbal, to work effectively with patients,
families, and healthcare providers. Knowledge of community
resources and services available to patients. Ability to adapt to
different healthcare settings and work with diverse patient
populations. Preferred: BSN (Bachelor of Science in Nursing) or MSN
(Master of Science in Nursing). Certification in Case Management
(CCM) or related certifications. Experience in discharge planning,
utilization review, or care coordination. Previous travel nursing
or multi-facility experience. Working Conditions: Travel required
to different healthcare facilities on a temporary basis. Ability to
work independently with minimal supervision. Flexibility in working
hours, including evenings, weekends, and on-call shifts as needed.
Exposure to various clinical environments and patient populations.
Occasional physical demands, such as assisting with patient
mobility, lifting case files, and standing for extended periods.
